What cosmetic dentistry actually covers

It is a small set of tools, and the skill is choosing the smallest one that will do the job. Whitening lightens the natural tooth from within. Reshaping smooths an uneven edge by a fraction of a millimetre. Bonding adds tooth-coloured material to a chip. Veneers are thin shells cemented over the front of a tooth, and unlike the others they need enamel shaped away first. Minor gum contouring can help where the gum line is uneven.

Smile design is the thinking that comes first: how your front teeth relate to your lip line, the midline of your face, and what shows when you speak. Teeth that suit one face look borrowed on another, which is why we do not work from a catalogue.

Planning here is by Dr. Kusha Dhawan Mehta, a prosthodontist, trained in rebuilding tooth surfaces so they are hard to spot.

What happens, step by step

Nothing irreversible happens at the first visit, or the second.

  1. Consultation and records, Visit 1 (about 45 minutes)

    We ask what specifically bothers you, because "I do not like my smile" and "this tooth is chipped" lead to different plans. Then an examination of teeth, gums and bite, and photographs of your face at rest and smiling. [Confirm: whether digital smile design photography or software is used at the clinic.]

  2. Health first, then appearance

    Decay is treated and gum inflammation controlled before cosmetic work is planned. Work placed over inflamed gums or hidden decay looks fine for a few months, then declares itself, by which point the tooth underneath has lost more ground.

  3. The plan, gentlest option first, then a trial

    You see the options in order of how much of your own tooth they spend, and veneers come last on that list, not first. Where the shape or length of front teeth changes, a trial is usually made: a mock-up placed over the teeth with no drilling, or temporaries you wear. Changes are easier now than after.

  4. The work, then a review

    Whitening is done over [Confirm: in-clinic session, take-home trays, or both]. Reshaping or bonding is usually one appointment; veneers and crowns take two or three visits while the laboratory makes them. [Confirm: in-house or external dental laboratory.] You return so anything that feels off in the bite is adjusted.

The honest answers

This section exists because cosmetic dentistry is easy to oversell.

Healthy teeth should not be ground down to look better

A healthy front tooth with intact enamel is better than any shell we can make for it, and enamel is not replaceable. Preparing sound teeth for veneers because they are a shade darker than someone else's photograph commits you to replacing those veneers for life, and to teeth that can never stand alone again. "My teeth are fine but I want them brighter" is not a reason to remove enamel.

Whitening has limits, and does not touch crowns or fillings

Whitening works on natural enamel and dentine. It does nothing to porcelain, ceramic or white filling material, which keep the shade they were made in. Whiten around an existing front crown or filling and that restoration can look darker by contrast, and matching it means replacing it. Whitening fades too, so it is a maintained result, not a permanent one.

Veneers are irreversible, and that word is used precisely

Enamel is shaped away to make room for the shell. Once it is gone the tooth is committed, and veneers do eventually chip, stain at the margin or come loose and need remaking. We are not against veneers; we are against agreeing to eight of them in a first appointment.

Sometimes the answer is a clean and a small reshape

Plenty of people who come in asking about veneers leave with a thorough cleaning, whitening and a fraction of a millimetre off one uneven edge, and they are content. Starting small keeps every option open; starting with veneers closes most of them.

We will not promise a particular outcome

What we can do is show you the trial, explain the limits of your own teeth, and be specific about what will and will not change. Photographs of other patients are a poor guide: different teeth, different gums, a different face.

Looking after cosmetic work

Cosmetic dentistry is maintained, not finished.

The first two weeks

After whitening, teeth take up stain more readily for a few days, so go easy on tea, coffee, red wine and tobacco. After bonding or a new veneer, avoid biting hard things with that tooth. Mild cold sensitivity is normal and settles.

Every day, ongoing

Brush twice and clean between the teeth once, with particular attention to the gum margin where a veneer or crown meets the tooth, because that is where trouble starts. If you grind at night, wear the night guard.

Realistic lifespan

Whitening around 1-2 years before a top-up. Bonding a few years, then repolishing or replacement. Veneers and crowns often 10-15 years or more, sometimes less under heavy load. Check-ups catch a loose margin early.

Why plan a smile with a prosthodontist?

Prosthodontics is three years of training beyond a dental degree spent on crowns, veneers, bridges, dentures and how they sit in a real bite. That training is as much about knowing when not to place something as how to place it.

Front teeth are not just seen, they are used. Work that looks right but cannot chew, or chips within a year, is not a success.

Cosmetic questions, answered straight

Will whitening work on my teeth?
Sometimes yes, sometimes only a little. Whitening lifts stains within natural enamel, so teeth darkened by tea, coffee, tobacco or ageing usually respond. Teeth grey from an old injury or banded from childhood medication respond much less. And whitening does not change crowns, veneers or white fillings at all, so whitening around an existing front restoration can leave a visible mismatch. We plan the order carefully, or advise against it.
Are veneers reversible? Can I change my mind later?
No, and it is the most important thing to understand before agreeing to them. Enamel is shaped away to make room for the shell, and enamel does not grow back. From then on that tooth always needs something covering it, and the veneer itself will need replacing at some stage. That is a fair trade for a tooth already chipped, worn or badly discoloured. It is a poor trade for a healthy tooth that is simply a shade you dislike.
I do not like my smile but my teeth are healthy. What are my options?
Often more, and gentler, than you expect. Cleaning and stain removal, whitening, smoothing a jagged edge by a fraction of a millimetre, or bonding a chip can change how a smile reads without removing healthy tooth structure. Straightening moves teeth without touching their surface at all. We work through the reversible options first, and only discuss veneers if the gentler ones cannot do the job.
Will people be able to tell?
That depends on how the work is planned, which is why we start with your face rather than a photograph of somebody else's teeth. Teeth that are all one flat shade, all one length and brighter than the whites of your eyes tend to read as artificial. Natural teeth vary in shade, texture and edge shape, and they relate to your lip line and the way you smile.
Why do my gums need treating before cosmetic work?
Because gums are the frame around every tooth, and they move. Inflamed gums shrink back once treated, so a margin that looked correct during treatment can end up with a visible line at the edge. Untreated decay under a new veneer is worse, it keeps progressing out of sight. So gum treatment, cleaning and any fillings come first. It adds a few weeks, and the work then sits on something stable.
How long does cosmetic work last?
Whitening typically holds around 1-2 years, less with heavy tea, coffee or tobacco use, and it can be topped up. Bonding on a chip lasts a few years, then it is polished or replaced. Well-made veneers and crowns often serve 10-15 years or more, but they can chip or come loose, particularly in someone who grinds at night.
